Could someone tell me what I have done wrong to this assembly?

I have it fully assembled at least it seems however I cannot create an exploded view to match my requirements for my homework. I needed to create an exploded view and collapse it like I have done in the past in order to make a drawing that represents it.

I will attach my piece then the layout of how it should be formed and how many of each part if comparing my piece would help
