How to make bends on a cylinder in Solidworks?

Hi,
I am beside myself trying everything I know to build this shape into a sheet metal part. I have drawn it a as a part file an have built it out of brass shim stock.
Part thickness 0.005"
Can anyone help or give advice.

You need a flat zone wherever you need to place a bend out of the main cylinder.

In the attached pic, all the highlighted areas are 'planar' chord segments embedded in the overall curvature of the part.
